2012 MEETING REGISTRATION 

The registration are now available. There are 2 ways to register. You can download form 1, print it and fill it out longhand. The better method is to download form 2. This form is "fillable" using Adobe Reader. It calculates the fees and totals them for you. It will also calculate the total charged to your credit card if you choose to pay by that method. These forms were designed with the latest version of Adobe Acrobat. In order for you to efficiently use them you will need the latest version of Reader. It is availabe as a free download from Adobe. The link to Adobe Reader has ben provided. Select the right version for your computer operating system, and download. If you attempt to use the fillable version without Adobe Reader, you will be dissapointed. Once completed, Save, Print, Sign, and mail the completed form to Dr. Keene. Be sure to keep a copy for your records. Some versions of Reader will not save a "completed" form, only a blank version.
